Lorsqu'un individu revêt le costume du Ghostface et s'attaque à la production du film STAB 3, Sydney Prescott sort de sa cachette et part pour Hollywood afin d'éviter un nouveau massacre. Ce nouveau maniaque souhaite un "Acte final" totalement tordu dans un jeu insidieux ayant pour seul but de briser toutes les règles de l'horreur...
